For cosmetic surgery and medspa use only. No points or rewards are given for reconstructive (insured surgeries) or services. We reserve the right to discontinue the use of the platform at any time. Accrued points have no cash value and many not be redeemed outside of the platform for cash or other compensation. Terms and Conditions may change and individual activities have their own appropriate Terms and Conditions that the user must accept. User understands that this platform is social and sharing in nature and participating is a public action. JW will never share procedure information however it is really important to be mindful of interactions on social platforms and your own privacy settings.

The following rules are intended for viewing in the 50 United States and the District of Columbia (excluding New York and Florida) by legal residents thereof only. Do not proceed in this site if you are not a legal resident of, and located in, the 50 United States or the District of Columbia (excluding New York and Florida) at the time of entry. Residents of New York and Florida are not permitted to enter or win. Persons who are prohibited from receiving Botox cannot enter or win.

This sweepstakes is sponsored by Chicago Center for Plastic & Reconstructive Surgery, LLC d/b/a JW Plastic Surgery ("Sponsor"), 20 W Ontario Street, Chicago, IL 60654.

1. HOW TO ENTER. Starting at 12:01 a.m. on Sunday, September 10, 2018, each calendar week through 11:59 p.m. on Saturday, September 10, 2019 (the “Promotion Period”), you may submit one entry per draw action per calendar week by either submitting a complete official entry at the tab found at https://www.facebook.com/JWPlasticSurgery/ (the “JW Plastic Surgery Facebook Page),”liking” the JW Plastic Surgery Facebook Page, or submitting an entry via Instagram using the hashtag #jwplasticsurgery or submitting through the Social and Loyal Program Platform, you may win 15 units of Botox and related professional services with an approximate retail value of $150. By submitting an entry on the JW Plastic Surgery Facebook Page or “liking” that page, the Social & Loyal Platform, or entering through Instagram using the designated hashtag, by completing the specified draw action you are automatically entered into this contest. Only one entry permitted per week per draw action, regardless of social media channel. Entries will not carry over into following weeks but entrants may make one (1) new entry per week per draw action. Entrants give their express permission to be contacted by Sponsor by social media, text, e-mail and/or postal mail, but shall have the ability to opt out of receiving such materials at any time. Odds of winning depend on number of entries received. There will be one (1) winner per week for the fifty-two (52) weeks of the Promotion Period.

Entries will be deemed to be made by the authorized Facebook account holder of the JW Plastic Surgery Facebook Page or Instagram account holder at the time of entry. This promotion begins Sunday, September 10, 2018 and ends on Saturday, September 10, 2019. By entering, entrants agree to be bound by the terms of these Official Rules. Free Internet access and e-mail accounts for on-line registration and entry may be obtained from a variety of sources, including public libraries and certain web sites such as www.yahoo.com and www.google.com. Sponsor has no affiliation or relationship with any of these organizations, and makes no representations or warranty of any kind with respect to the services offered by these organizations. Sponsor is not endorsing any source of free Internet access and/or email accounts, including, without limitation, those sources identified above.

2. ELIGIBILITY. You must be a natural person, of the age of majority in your state of residence, and a legal resident of the 50 United States or the District of Columbia (excluding New York and Florida) for the duration of the promotion, with a valid e-mail and postal mail address. Employees of the Sponsor, its subsidiaries, affiliates, sales representatives, distributors, advertising and promotion agencies, and members of their immediate families (spouses, parents, children, and siblings and their spouses) and/or those living in the same household of each of the above are not eligible to participate. By entering, all entrants acknowledge and agree that their name and state of residence may be posted at https://www.facebook.com/JWPlasticSurgery/ and https://www.instagram.com/jwplasticsurgery/ and grant their permission to have their name and state of residence posted there and their name included on the official winners’ list.

3. WINNERS. Proposed winners will be randomly selected at the Sponsor’s offices, and Sponsor’s decisions are final and binding in this and all other matters related to this promotion. The proposed weekly winner for the week just ended will be notified via e-mail by Sponsor on or before the next business day. Sponsor will attempt to contact the proposed winner once via the email address provided at the time of entry or via direct message on the social media platform utilized for entry. If there is no response to the communication within five (5) days, Sponsor will select a new proposed prize winner by random draw from among the remaining eligible entrants for the week in question. An entrant is not a winner of any prize unless and until entrant’s eligibility has been verified and entrant has been notified that verification is complete.

4. PRIZE.

One (1) proposed winner per week for the 52 weeks of the Promotion Period will receive a certificate for 15 units of Botox or related professional services with approximate retail value of $150.00, redeemable only at JW Plastic Surgery within three (3) months after prize notification. The certificate cannot be combined with other specials or offers. Certificate will be sent by postal mail within seven (7) days after winner’s eligibility is verified.

Any difference between actual costs and actual retail value will not be awarded. Prizes cannot be exchanged for cash. No prize transfer, assignment, or substitution permitted except by Sponsor. If prize becomes unavailable, Sponsor reserves right to substitute prize for a prize of equal or greater value. Subject to suitability for service, to be determined by Sponsor based on an in-person evaluation of winner. If prize is medically inappropriate for winner, winner may choose a medically appropriate service of similar value at Sponsor’s practice. No more than the number of prizes set forth in these Official Rules will be awarded. All prizes will be awarded. Only official certificates (no facsimiles or other copies) will be honored.

5. TERMS AND CONDITIONS. All taxes (federal, state, or local, if any) and other expenses associated with the acceptance and/or use of any prize are the sole responsibility of the prize winner. Prizes are not redeemable for cash and are non-transferable. By accepting prize, winner grants Sponsor and its subsidiaries, affiliates, sales representatives, distributors, advertising and promotional agencies ("Sponsor and its Agents"), the right to use and publish his/her name, information obtained from entry form, state of residence and/or likeness for advertising and promotional purposes, online and in print, or in any other media worldwide, in connection with the promotion, without limitation and without additional compensation. Noncompliance with these requirements, or the return of any prize/prize notification as undeliverable, may result in disqualification and selection of an alternate winner. Sponsor will not replace a lost or stolen prize.

The names, addresses, and other information regarding contestants may be retained and used by Sponsor and its Agents for customer list and market evaluation.

6. OTHER. Sponsor and its Agents are not responsible for any human errors, any computer, online, telephone or technical malfunctions or delays of any kind that occur in the transmission or processing of entries, or for inaccurate transcription of entry information or for on-line entries that are stolen, misdirected, garbled, lost or delayed for any reason. If, for any reason, the contest is not capable of being completed as planned, including, but not limited to infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or other causes beyond the control of Sponsor, Sponsor reserves the right at its sole discretion to cancel, terminate, modify or suspend the contest and to choose winners from among the eligible entrants prior to cancellation, termination, modification or suspension of the contest. Sponsor also reserves the right in its sole discretion to disqualify any entrant who circumvents, violates or fails to comply with these Official Rules, who attempts to enter this contest in any manner or through any means other than as described in these Official Rules, who attempts to disrupt this contest, or who acts with the intent to annoy, abuse, threaten or harass any other person.

7. GENERAL RESTRICTIONS. This sweepstakes is governed by the laws of the state of Illinois without regard to the conflicts of law provisions therein, and, by entering this sweepstakes, each entrant submits to the exclusive personal jurisdiction and venue of the state and federal courts of that state. All federal, state and local laws and regulations apply and this sweepstakes is void where prohibited or otherwise restricted by such laws, including but not limited to jurisdictions with laws that would require registration, disclosure, filing, trust account, or posting of a bond or any other requirements that are not satisfied by these terms and conditions, including those jurisdictions which would not give effect to the choice of law and consent to jurisdiction provisions contained in the preceding sentence. The sweepstakes is not to be used in connection with any form of gambling.

8. WINNERS IDENTIFICATION.
